Fibroid Embolization and Fertility

Problematic uterine fibroids are a common cause of infertility and pregnancy related complications. Interventional Radiologists are often asked what impact fibroid embolization can have on fertility.  A common concern is whether the embolization procedure will impact fertility negatively.  The exact impact that fibroid embolization has on future pregnancies remains somewhat uncertain as women who present with fibroids and primary infertility are often older and therefore are more likely to have pregnancy complications due to their age and other medical comorbidities. The impact of fibroid embolization is difficult to separate from the other underlying risk factors.

A study in 2017 looked at 359 women (average age 36 years) who had failed to conceive after at least 1 year of unprotected intercourse. The infertility was otherwise unexplained and presumably due to either presence of fibroids or adenomyosis. The chance of a live birth was 24% at 12 months and 37% at 48 months. It was the first pregnancy for 86% of women who conceived.  The study proves that it is possible to have a baby after fibroid embolization even in the setting of infertility.  It did not however demonstrate what effect if any embolization has on future pregnancies and did not compare embolization to other treatments such as myomectomy.
